It's web application that serves to simplify the procedure for preparing and printing applications for specific permissions. Automatically enforces providing data that is needed by persons verifying a given application.

After completing the Generate command, the completed from is sent to the e-mail address of the person completing the application.

The document prepared in this way should be printed.

It must then be legibly signed by the persons indicated on the application (e.g. the person submitting the application, the applicant't supervisor, substantive persons responsible for the given permissions), and then it must be delivered to the appropriate unit/person (application addressee).

ATTENTION! Applications containing formal errors, incomplete information or sent to the wrong recipient may be sent back to the sender in order to improve, complete formalities or select the approproate unit responsible for the implementation of a given action.
